Dear Library Directors,
The FY2013/2014 Public Library Annual Report/State Aid Application will close today at 11:59:59 pm.  This is the final chance to submit your report.  Please make sure to submit your Report by 11:59:59 TODAY and mail in your Certification of Information pages with original signatures (postmarked on or before February 5th).

After you have electronically submitted your Annual Report/State Aid Application, please send your signed Certification of Information sheets to:
