[Michlib-l] Patron replacement of books - same edition?

Hello all!
I have a few questions on how you handle patron replacement of damaged or
lost items.
1. Do you have a fixed price or are they allowed to replace it with a
new item?
2. Do they order the replacement item or do they cover your cost?
3. Do they have to replace it in the same edition (hardcover vs trade
paperback vs mass market)?
4. What do you charge as a processing fee to process it back into the
collection (staff time, mylar, barcode, and call number)?
On another note:
1. Do you have any patrons who will not check out trade or mass market
paperbacks because of the size of the print, but will check out hardcovers
that are not large print?
