[Michlib-l] DVD/CD damage/repair notice

Hello everyone,
We are curious to know if you use some sort of label or form in your DVDs
and/or CDs that encourages patrons to inform you of a disc that is
damaged/skips/etc.  If so, would you mind sharing a sample of the
form/label and how you use it?  Thanks!
