[Michlib-l] Book Ordering

Greetings!
Acquisition people, what is your ordering procedure; specifically if you
have more than one selector. We currently use cards that ask for all
pertinent information. We tried to do an online google form and that was a
disaster. Is there some program out there that I don't know about?
