[Michlib-l] Volunteer Release and Waiver of Liability form??

Hello library knowledge bank,
Our library is working to develop a home delivery program. We're planning
to use our volunteer base to make the deliveries like Meals on Wheels, but
with library materials. Does anyone have a Volunteer Release and Waiver of
Liability form they could share?
