As a program participant, I want to mention how much our community appreciates having free COVID tests available at the library. We've made thousands of tests available to the community at no charge since we enrolled in the program. Very easy to participate and requests additional tests as well. Highly recommend it!

Due to heightened interest in the Library Partnership for At-Home Test Program, the Michigan Dept. of Health and Human Services has asked us to disseminate this request to you:





 MDHHS Library OTC COVID-19 Test Distribution Program

The Michigan Department of Health and Human Services (MDHHS) wants to ensure all libraries in the State of Michigan are made aware of the Library Partnership for At-Home Test Program.



This voluntary program aims to increase access to at-home COVID-19 test kits for individuals, families, and communities who may otherwise struggle with access to healthcare resources,

be hesitant to visit a COVID-19 testing location or may lack the funds necessary to purchase at-home test kits for themselves or their families.



Libraries that are interested in the Library Partnership for At-Home Test Program:

  *   Must enroll using the COVID Self-Test Distribution Site Enrollment Survey. https://forms.office.com/Pages/ResponsePage.aspx?id=h3D71Xc3rUKWaoku9HIl0ZGHyz3HKppAusICATcUVNlUQVg2NFZRR1JQR1BRTkJDMTNSWjlINzc0VCQlQCN0PWcu
     *   Following enrollment, libraries will receive a program overview and a link where they can place orders for test kits.

If your library is currently enrolled and you wish to no longer participate, please email your request for removal to MDHHS-COVIDTestingSupport at michigan.gov<mailto:MDHHS-COVIDTestingSupport at michigan.gov>.
